On 11/4/2025 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to conduct a health and safety check. LPA was allowed into the facility. LPA met with Administrator Tina Healy & Ron Cordy Program Director, LPA explained the reason for the visit.
The following documents were provided at the facility for R1:
Staff training for staff on duty during the incident
Care Plan - Administrator stated care plan was not completed
LPA will receive the following documents on 11/5/2025 by 12:00 PM.
Exit interview was conducted and a copy of this report LIC809 was provided to Administrator Tina Healy.
