The inspector’s account

On 04/21/26, Licensing Program Analyst (LPA) J. Duarte arrived unannounced to conduct an Annual Inspection. There was no answer at the door. LPA contacted Administrator Misty Vease, via telephone and she stated that she would arrive shortly. The administrator arrived shortly after. LPA introduced self, stated the purpose of the visit, and was granted entry. The administrator stated that all resident were at day program.

The facility was observed to be at a temperature of 70 degrees F, clean with no passageway obstructions The living room has sufficient seating for residents. The kitchen was toured and LPA observed a two-day supply of perishable and seven-day supply of non-perishable food. The facility stores sharps in a locked kitchen cabinet. A fire extinguisher was observed in the dining room with a service date of 01/23/2026.

All bedrooms were toured and observed to have the required furniture and adequate lighting. The hot water temperature was tested and observed at 109 degrees F in the hallway restroom and 107 degrees F in the master bathroom.

The facility has a laundry room with a washer and dryer. Hygiene products are stored in the laundry room. LPA toured the garage. Chemicals and detergent were observed locked in cabinet. The facility has an additional supply of food and water stored in the garage.

Outside of the facility was toured and observed to be free of debris. The facility has a back patio with seating available. The side gates are self-latching and self-closing.
