03/14/2025 and conducted by Evaluator Jacques Leffall
COMPLAINT CONTROL NUMBER: 24-AS-20250314124118
Staff overmedicated a resident in care resulting in death of resident.
On 9/26/25 at 12:30 pm Licensing Program Analyst (LPA) J. Leffall conducted a subsequent complaint visit to deliver findings on above allegation. LPA met with House Manager (HM) Jethro Pandy and stated purpose of visit.
The department conducted interviews and reviewed records. Based on the information obtained, the above allegation is unsubstantiated.
Exit interviewed conducted. A copy of this report was provided to Administrator whose signature confirms receipt of the report.
