The inspector’s account

On 3/27/2025, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required Inspection. LPA introduced self, stated purpose of visit and allowed entrance by Administrator, Daniel Esparza,

Currently there are three (3) individuals residing in facility, all were at day program at time of inspection. Day program hours are Monday through Friday 9:00AM - 2:30PM. Facility is a four bedroom, 2 bathroom home, with all private bedrooms. Facility observed to be clean, well lit, odor free, and a comfortable temperature. Resident tour conducted with Administrators Daniel Esparza and Eric Coronado. Facility tour started in bedrooms, all bedrooms observed to have required furnishings, all bedding observed to be in clean and in good repair. Bathroom toured, shower observed to have a shower chair, grab bar and non-skid mat available. Water temperature measured at 115 degrees F. Living room and activity room have adequate seating available for all residents. Kitchen toured, LPA observed a 2-day supply of perishable food and a 7-day supply of non-perishable food available. All knives were observed to be locked and secured in a kitchen drawer. Medications observed to be locked and secured in a medication cart and inaccessible to residents. Medications observed to have original labels and to be administered as prescribed.

Fire extinguisher present with a service date of 11/06/2024. Carbon monoxide and smoke detectors present and observed operational during inspection. Last fire drill was conducted 3/5/2025 and last disaster drill was conducted 3/03/2025 according to facility records. Chemicals observed to be locked and secured under kitchen sink and in laundry room cabinet.

Outside of facility toured. Designated fire exit opens free of obstruction. Facility has shaded area and seating available for individuals. No hazards observed.
