Facility Evaluation Report

Licensing Program Analyst (LPA) Shawna Doucette arrived at the facility to conduct a case management regarding an incident that occurred on 7/4/24 resulting in a death. LPA met with Administrator Sophia Nelson.

The Department received an incident report indicating C1 became unconscious while swimming in the pool. Staff called 911 and C1 was hospitalized. The Department was later notified C1 passed away. Facility staff had CPR and First Aid certificates. Facility staff did not have the water safety course certificate.

Based on the review conducted by the Department and information gathered, the following deficiency was cited on LIC 809-D

An exit interview was conducted, and a copy of this report along with Appeal Rights (LIC 9058) and plan of correction was provided whose signature below confirms receipt of these rights.
