07/29/2025 and conducted by Evaluator Jimmy Duarte

COMPLAINT CONTROL NUMBER: 24-AS-20250729164726

On 08/07/2025, Licensing Program Analyst (LPA) J. Duarte conducted an unannounced visit to conduct a complaint investigation. LPA introduced self and stated the purpose of this visit and allowed entrance. LPA met with Administrator, Ulysis Baal during this visit.

This Department investigated the allegation of resident was illegally evicted. Administrator Ulysis advised that the resident has not been evicted and the resident is currenlty residing in the facilty. LPA observed the resident in the facility.

This Department has found that the above allegation is UNFOUNDED, meaning the allegation is false, could not have happened, and/or is without reasonable basis. We have therefore dismissed the complaint.
