Facility Evaluation Report
On 9/23/2025, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required inspection. LPA identified self, stated purpose of visit, and allowed entrance by direct care staff. Licensee/Administrator Dennis De La Cruz contacted by telephone and arrived during inspection to meet with LPA.
Facility observed to be clean, odor free, and a comfortable temperature. LPA observed common areas of the facility to have adequate seating and lighting available. Residents observed to be watching television in the living room and some relaxing in their bedrooms. Kitchen toured, LPA observed facility to have a 2-day supply of perishable and a 7-day supply of non-perishable food available for residents. Resident bedrooms toured, all bedrooms observed to have required furnishings available. Resident bathrooms toured, all fixtures observed operational, shower observed to have skid resistant mats, shower chair and grab bars available. Hot water measured at 114 degrees F.
Facility is equipped with 2 fire extinguishers with a purchase date of 9/15/2025. Smoke detectors and carbon monoxide detectors tested and observed operational at time of inspection.
Outside area toured, fire exit observed to be self-latching. Facility has covered patio with seating for residents. No hazards observed.
Due to time constraints, LPA will return to complete inspection tool and review both resident and staff file reviews.
