06/13/2022 and conducted by Evaluator Shawna Doucette

COMPLAINT CONTROL NUMBER: 24-AS-20220613163903

Licensing Program Analyst (LPA) Shawna Doucette contacted the facility to commence a complaint investigation and deliver findings. LPA identified herself and discussed the purpose of the visit and the elements of the allegations with Administrator John Nobleza.

The Department has investigated the allegation: Staff abandoned resident at hosptial

Based on record review and interviews, R1 was was sent to the hospital and during hospital visit required a higher level of care. R1 developed a prohibited health condition. We have found that the complaint was UNFOUNDED, meaning that the allegation is false, could not have happened, and/or is without a reasonable basis, therefore we have dismissed the complaint.

An exit interview was conducted and a copy of this report was provided.
