Facility Evaluation Report
On 02/02/26, Licensing Program Analyst (LPA) M. Yang arrived to conduct an unannounced complaint
investigation and met with Licensee (L1) John Nobleza.
During the course of the investigation, LPA toured the facility. At approximately 01:08PM, L1 and LPA observed in R1’s drawers in room 1 multiple medications belonging to Staff 1 (S1) unlock. At approximately 01:14PM, LPA and L1 observed R2 sleeping in bed when R2’s medications were observed unlocked in the resident’s drawers. At approximately 01:23PM, LPA and L1 observed S1’s medication unlocked in bathroom shelf and S1’s medication creams unlock in bathroom drawers.
A deficiency is being cited on the attached 809D in accordance to California Code of Regulations, Title 22,
An exit interview was conducted. A copy of this report and appeal rights was provided to Licensee, whose signature on this form confirms receipt of this report.
