Facility Evaluation Report

On 07/17/2026, Licensing Program Analyst (LPA) J. Duarte, completed an unannounced case management visit. LPA met with Administrator (AD), Tisa Williams, and explained reason for visit.

This case management is being completed for an incident that occurred on 05/04/2026, with allegations that the day program and R1's residential staff were placing drugs in R1's food.

During today's visit, LPA interviewed staff. LPA obtained the following documents for R1: Admission Agreement, Physician Report, IPP, Individual profile/Emergency ID, and Action Plan.

LPA conducted a second case management for an incident with employment services that occurred on 03/28/2026, where a staff fell asleep at the job site. LPA interviewed staff and requested the Individual Service plan and IPP for R2.

At this time, these case managements will need further investigation. AD was informed that the obtained documentation will be reviewed and additional interviews or visits may be needed.
