Facility Evaluation Report

Licensing Program Analyst (LPA) J. Duarte conducted an unannounced Annual inspection visit. LPA met with Program Director Tisa Williams and Area Director Hanh Mireles arrived shortly after. LPA discussed the purpose of the visit.

LPA and the Program Director toured the facility together. Facility was maintained at a temperature of 71 degrees F. LPA observed adequate seating and lighting throughout the facility. No passageway obstructions or hazards were observed inside or outside. Client restrooms were toured, observed to be clean, and operational with adaptive devices. The restroom hot water measured at 119 degrees F. Facility dining area was toured. Clients bring their own lunches to program. Locked cubbies were observed in several areas. Cleaning supplies were observed behind locked door, in a storage room.

LPA toured the Art Room, Sensory Studio, Recreation Room, Computer Room, Game Room, Library, Relaxation Room, and Learning Room. LPA observed clients in these rooms having a morning snack and actively engaged in their activities with staff supervising.

Medications were observed to be locked and secured in the directors office. LPA reviewed a sample of medications and the MARS and per records, medication is being administered as prescribed.

Facility extinguishers present throughout facility with a service date of 10/23/24. Facility is equipped with pull stations, strobe lights, sprinklers. Fire sprinkler system was inspected by the State Fire Marshall on 7/15/25, per staff records. Carbon monoxide detectors and smoke detectors present. First Aid kits present and observed to have all required items. Last fire drill was conducted on 08/29/25, per staff records.
