Licensing Program Analyst (LPA) conducted an unannounced Case Management visit in response to a report provided by the facility. LPA met with Administrator Jeanette Madkins and discussed the purpose of the visit.
LPA met with Regional Director, Program Director, and Administrator and conducted interviews.
LPA Williams also interviewed Staff 1 in regards to another Case Management.
At this time further information is required.
