Facility Evaluation Report
On 03/06/2026, Licensing Program Analyst (LPA) J. Duarte arrived unannounced at the above facility to conduct Annual Inspection. LPA introduced self, stated the purpose of the visit, and was granted entry to the facility by Administrator Gonzalo Cruz. Administrator stated there are four residents, two residents were at day program, one resident was on a family outing, and one resident was home. LPA toured the facility with the Administrator.
The facility was observed to be at 72 degrees F. No passageway obstructions were observed. The common areas were well lit with adequate seating for residents. The carpet was observed to be dirty throughout the facility and resident rooms. LPA toured the kitchen. The facility had a two-day supply of perishable and a seven-day supply of non-perishable food. LPA observed the kitchen had broken counter top tiles.
The First Aid Kit was observed in the dining room and had all required supplies. The dining room window had a ripped window screen. The hallway restroom hot water measured at 109 degrees F. This hallway restroom was observed to have ripped linoleum flooring. The restroom hot water attached to the room measured at 107 degrees F. Resident bedrooms were observed to have required furnishings. LPA observed that R1's room had a hole in the wall and the window frame in the interior had dirt.
The facility conducts their own laundry and has a washer and dryer. Detergent, chemicals, and sharps were observed locked in a cabinet in the garage. A freezer was observed in the garage with an additional supply of food.
