Facility Evaluation Report

Licensing Program Analyst (LPA) Darius Williams conducted an unannounced visit in response to a Decision and Order. LPA Williams met with Administrator, Nora Lopez and discussed the purpose of the visit.

LPA met with Administrator and she reported Staff 1 (S1) was never hired by the facility. LPA reviewed facilities current schedule and did not locate S1.

An exit interview was conducted and a copy of this report was provided.
