Facility Evaluation Report
On 07/09/2026, Licensing Program Analyst (LPA) J. Duarte, completed an unannounced case management visit. LPA met with Administrator (AD), Nora Rodriguez Lopez, and explained reason for visit.
This case management is being completed for an incident that occurred on 07/01/2026, with R1 being placed in a seated restraint.
During today's visit, LPA interviewed staff. LPA requested the following documentation for S1, S2, S3,S4, S5, and S6: Application, job description, Health Screen, Crisis Prevention Intervention Certification, and proof of training. LPA requested the following documents for R1: Admission Agreement, Physician Report, Appraisal, and Needs and Service Plan.
At this time, this case management will need further investigation. AD was informed that requested documentation will be reviewed and additional interviews or visits may be needed.
