Facility Evaluation Report
On 07/09/2026, Licensing Program Analysts (LPA) J. Duarte arrived unannounced to conduct an annual inspection. LPA J. Duarte explained the reason for inspection and was granted entry. LPA met with Administrator (AD) Nora Rodriguez Lopez and a tour was conducted. AD stated that residents are at the clinic or day program.
LPA toured the front desk reception area with AD Lopez. AD advised that the units have call buttons and a staff member is at the front station 24/7. LPAs toured a sample of six apartment units. All residents were away from the units toured. Each building was observed to have an evacuation chair under the stairs. Each building was observed to be at a temperature between 72 and 75 degrees F, clean, in good repair, with no passageway obstructions or fire hazards. LPA observed a seven day supply of non-perishable foods and a two day supply of perishable foods in apartment units. Fire extinguishers were observed in each apartment with a service date of 11/07/2025. All common areas were properly furnished and well-lit throughout. Medications, First Aid, and Sharp items are locked in each apartment in the kitchen cabinets. Smoke Alarms and Carbon Monoxide detectors in the units were tested and operational at time of inspection. The restrooms were equipped with grab bars next to the toilets and the showers, showers had skid resistant mats and shower chairs available, paper towels, and trash bins with lids. The restrooms hot water measured between 108 and 115 degrees F.
Medication, the Centrally Stored Medication Log, and MARs were reviewed for six residents and LPA. MARs log reflect medication is being administered as prescribed.
