Facility Evaluation Report

On 12/15/25, Licensing Program Analysts (LPA), J. Duarte and S. Doucette, conducted an unannounced Case Management (CM) regarding an incident report where a staff who is not a licensed professional assisted a resident that requires gastrostomy care, and caused the resident to require hospital care. LPAs met with Administrator Nora Rodriguez and discussed the purpose of the visit.

Based on interview and records, a deficiency is being cited on the attached LIC 809D page.

Plan of correction was reviewed and discussed with the Administrator.

An exit interview was conducted, and a copy of this report and appeal rights were provided to the administrator.
