Facility Evaluation Report

On 8/12/2025, Licensing Program Analysts (LPAs) J. Duarte and M. Medina conducted an unannounced Case Management visit. LPAs introduced self, stated purpose of visit and allowed entrance. LPAs met with Residential Manager/Administrator, Nora Rodriguez Lopez to conduct visit.

The purpose of this visit is to verify that Staff (S1) is not on the property. LPA provided a copy of Decision and Order to Administrator and Christina McGee, Human Resource Manager during visit, as a mailed copy was not on record. Both Administrator and Human Resource Manager understand that S1 is excluded and not permitted to be on the grounds.

Per Human Resource Manager, S1 has not been employed at facility since 3/10/2022.
