Facility Evaluation Report

Licensing Program Analyst (LPA) Darius Williams conducted a follow up pre-licensing visit. LPA Williams met with Licensee Anthony Barbato and discussed the purpose of the visit.

LPA Williams toured the facility and observed the fixes requested from the initial pre-licensing inspection on 3/29/2023. LPA Williams observed all items requested, to be in satisfactory condition.

Component III was reviewed and completed on 3/29/2023.

This report will be forwarded to the Central Application Bureau for further review.

An exit interview was conducted and a copy of this report will be provided via e-mail.
