Facility Evaluation Report

On 06/26/2026, Licensing Program Analyst (LPA) M Vega conducted an unannounced case management inspection to discuss Care and Supervision.

On 06/19/2026, Community Care Licensing CCL) received an incident report regarding two clients who had an altercation. Client 1 (C1) who hit Client 2 (C2). This was witnessed by staff after event happened. Staff observed there was an injury and appropriately notified the EMS for C2. Facility also notified CCLD.

Discussion was had regarding the appropriate response and to remind the staff to be vigilant in watching the client interaction and to take action when needed. Staff have been notified to speak with the residents, separate them and call appropriate parties if necessary. Administrator stated that both parties have been separated into different rooms to try and mitigate further incidents.

LPA obtained LIC 602, 625 and Client roster LIC 9020.

No deficiencies issued during this inspection. An exit interview was conducted with the Administrator
