Facility Evaluation Report

On 07/07/2026, LPA M Vega arrived at the facility unannounced to conduct an annual inspection. LPA was granted entry by Staff. LPA explained the purpose of the visit and met with Regional Director Steven Cruz and Administrator - Ty Scherer.

A tour of the facility was conducted with Administrator - Ty Scherer. The facility was set at 75 F temperature and free of passageway obstructions inside and outside. All residents’ bedrooms were observed to be at comfortable temperatures and have required furnishings. . Facility is free of debris and no passageway obstructions or fire hazards were observed. Common areas were properly furnished and well-lit throughout. LPA observed most residents in common area after breakfast watching television and others in their rooms resting. Department phone number and infection prevention information signs were posted thought the facility.

Resident, medication and staff records were reviewed. Records have needed information and are accurate at time of inspection.

Kitchen toured, supply of food observed and food stored properly for perishable and nonperishable. Medications were stored in a locked medication cart in office/medication room. Cleaning supplies were in a locked storage closet. Smoke detectors and carbon monoxide detectors were checked and operating. Facility had a pull station fire alarm. Facility has a fire sprinkler system. Fire extinguishers were charged and had service dates of 04/08/2026. LPA checked water temperature in resident bathroom which was 112.2 Degrees F.

There was outdoor seating for the residents, around the back they had shaded seating on one side of the facility and near the parking lot there was more shaded seating.
