Facility Evaluation Report

Licensing Program Analysts (LPA)'s Shawna Doucette and Sarah Hurt arrived at the facility unannounced to conduct a Case Management. LPA's met with Administrator Ty Scherer.

Facility fire sprinkler system wiring is damaged and sprinkler system did not pass inspection. Facility was made aware of the issue in 05/2025. Facility has contacted the company this week to make the necessary repairs. Administrator stated facility repairs are scheduled for tomorrow 07/17/25. Administrator stated the water will be shut off tomorrow. Administrator will email LPA a time frame as to how long the water will be shut off and will notify LPA and clients.

A copy of this report was provided with plan of correction and appeal rights to Administrator.
