The inspector’s account

On 5/18/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Inspection visit. LPA arrived, stated purpose of visit, and allowed entrance by Direct Care Staff. Licensee, Maria Emma Pangilinan contacted by telephone and arrived a short time later to conduct visit with LPA.

LPA observed facility to be clean, odor free and a comfortable temperature. Common areas observed to well furnished and have seating available for all residents in care. Resident bedrooms toured and observed to have required furnishings. Resident bathrooms observed to have grab bars, shower chairs, and slip resistant mats observed. Hot water measured at 108 degrees F. Kitchen toured, facility observed to have a 7-day supply of non-perishable and 2-day supply of perishable food. Medications observed to be locked and secured. Centrally stored medication logs, MARS and resident medication reviewed. Medication observed to have original labels and to be administered as prescribed.

Fire extinguisher present with a service date of 2/05/2026. Smoke detectors and carbon monoxide detectors observed operational during today’s inspection.

Outside of facility toured. Facility has a seating area with patio umbrella for residents. Side gate was observed to be self-latching. No hazards observed.

LPA requested the following documents be submitted to Fresno Regional Office no later than Friday, 5/29/2026: Personnel Report (LIC500), and Register of Facility Clients/Residents (LIC 9020).

No deficiencies observed during inspection. Exit interview conducted. A copy of this report provided to Administrator, Maria Pangilinan for facility records.
