Facility Evaluation Report
On 02/17/26, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct case management visit for the purpose of checking on the health and safety of the clients in care. LPA introduced self, stated the purpose of the visit, and requested to meet with Licensee Sheila Bagsby. LPA met with House manager Nelsalina Shabazz. Licensee was called and stated unable to attend meeting. LPA discussed the purpose of the visit with Licensee via telephone. Licensee authorized House manager to sign report.
LPA was informed by Licensee, that excluded individual was terminated in November 2025 and has not been associated to with facility since. Staff rooster (Lic 500) was received and confirmed excluded individual is not on schedule and not working at the facility.
No deficiency cited during visit. A copy of this report was provided to Licensee via email per requested. Exit Interview.
