Facility Evaluation Report
On 08/19/25, Licensing Program Analyst (LPA) M. Yang conducted case management-deficiency visit to the facility. LPA introduce self, stated the purpose of the visit, and met with Licensee/ Administrator Sheila Bagsby.
The purpose of the visit is to conduct a health and safety visit on the client in care and follow up on written incident reporting that was reported by the facility where S1 made multiple charges and cash withdrawals on C1’s ATM card. Copies of records were received, and interviews were conducted.
A deficiency is being cited, per California Code of Regulations, Title 22, Division 6, see attached Lic 809D.
An exit interview was conducted. A copy of this report and appeal rights was provided to Licensee, whose signature confirms receipt of this report.
