COMPLAINT CONTROL NUMBER: 24-AS-20220119122412

A Complaint visit was conducted on the date & times indicated above by Licensing Program Analyst (LPA) K. McClurg. LPA met with Executive Director IV (ED) Reg Webster.

Secondary tour from previous visit conducted. Area in wing of building B appeared to be clean & good in repair with no detectable stains &/or odors from pets. The Department has investigated the above allegation & determined it to be unsubstantiated.

Exit interview conducted with ED. Report provided.
