Facility Evaluation Report
On 5/21/2025, Licensing Program Analysts (LPA) M. Medina conducted a Case Management visit. LPA introduced self, stated purpose of visit and met with Executive Director/Administrator, Jeffrey Toomer and Sarah Archuelta-Weaver to conduct visit. .
LPA conducted Case Management information after Department received a Death Report (LIC 624A) on 5/20/2025 for Resident R1. Facility tour conducted during annual inspection on 5/14/2025. There are twenty-one (21) residents on hospice, and twenty-two (22) residents currently receiving home health services.
LPA received copies of R1's last physician report and facility progress note.
No deficiencies observed or cited during visit.
