Facility Evaluation Report
On 5/14/25, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Inspection. LPA introduced self, stated purpose of visit, and allowed entry. LPA Medina conducted facility tour with Executive Director, Jeff Toomer and Sarah Archuleta-Weaver, Health and Wellness Director.
Facility observed to be clean, odor free and a comfortable temperature. Residents observed throughout the community in both buildings in dining areas, and common lounge areas participating in various activities. Facility tour included both Building A and Building B dining rooms, facility has adequate seating available for all residents. Kitchen toured, facility receives food delivery 2 times per week. LPA observed a 2-day supply of perishable and 7-day supply of non-perishable food available as well as Emergency Supply food. Daily menus are posted in dining areas with alternate options available.
LPA toured resident bedrooms in Assisted Living, Memory Care and Independent Living. The following resident apartments were toured in Building B #100, #107, #128, #134, #201, #216, #229, #236, #239 and #250, water temperature measured and ranged from 110 degrees F. to 114 degrees. The following resident apartments were toured in Building A #112, #122, #201, #316, #325, water temperature measured and ranged from 108 degrees F to 110 degrees F. LPA observed grab bars and non-skid surfaces in all of the resident bathrooms toured.
Facility is equipped with fire pull stations throughout both buildings. Fire extinguisher all have current service dates of 01/2/2025. Carbon monoxide detectors observed operational during today's visit. Last fire drill conducted on 4/29/25, according to facility records.
Due to time constraints, LPA will return to review resident and staff files and complete inspection tool.
