Licensing Program Analyst LPA conducted a Case Management to follow up on an incident report that occurred on 5/23/22. LPA was met by Staff Karen Lomax and discussed the purpose of the visit. Administrator Reg Webster responded to assist with the case management.
LPA obtained copies of R1's file. LPA interviewed staff.
An exit interview was conducted with Administrator Reg Webster a copy of this report was provided.
