Facility Evaluation Report
On 6/27/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required inspection. LPA arrived, introduced self, and allowed entrance by direct support professional (DSP), also present was Administrator, Sharon Thomas.
Currently, there are four (4) residents in care. Facility is a 4 bedroom, 2 bathroom home with private bedrooms. All residents were present at time of inspection and observed to be relaxing watching television. Facility tour conducted with Administrator. Facility observed to be clean, in good repair, and a comfortable temperature. Resident bedrooms toured, all bedrooms observed to have required furnishings available. Resident bathrooms toured, shower area observed to have grab bars and shower chair available, all fixtures observed to be operational. Water temperature measured at 105 degrees F. All common areas observed to have adequate seating available for residents in care. Kitchen toured, facility observed to have a 2-day supply of perishable food and a 7-day supply of non-perishable food available. All knives observed to be locked and secured in the medication closet. Medications and MARS reviewed, all medication observed to have original labels and to be administered as prescribed.
Outside of facility toured, facility does have a seated area with shade available for resident seating. Exits observed to be free of obstruction, no hazards observed.
Smoke detector and carbon monoxide detector observed to be operational at time of inspection. Fire extinguisher present with a service date of 8/06/2025. Last fire drill was conducted on 6/10/2026 according to facility records. Staff and resident files.
No deficiencies cited during inspection. Exit interview conducted and a copy of report provided for facility records.
