Facility Evaluation Report

On 11/29/2025, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required Inspection. LPA arrived, introduced self, and stated purpose of visit. LPA met with Licensee/Administrator, Eric Lopez to conduct inspection.

Currently, three (3) residents in care, all residents were president during today's inspection and observed to be relaxing at start of inspection. There is currently one resident on hospice and one resident receiving on receiving home health services.

Facility observed to be clean, odor free, and a comfortable temperature. Facility tour began with the resident bedrooms. Resident bed rooms are adequately furnished and have sufficient lighting. Resident bathrooms toured, bathrooms observed to have grab bars in all toilet and tub/shower areas with skid resistant mats available. Water temperature measured at 113 degrees F. All common areas have sufficient seating and lighting for all residents in care. LPA toured kitchen, LPA observed a 2-day supply of perishable and a 7-day supply of non-perishable food available. All food observed to be properly stored and dated. Medication observed to be locked and secured in cabinet in laundry room. Detergents, cleaning supplies are locked and secured in both laundry room and garage. Sharps observed to be locked and secured in kitchen drawer.

Fire extinguisher has a service date of 10/14/2025. Smoke detectors and carbon monoxide detectors present and observed operational during inspection. Last fire drill conducted 10/05/2025 according to facility records.

Outside of the facility was toured. All exits open freely and free of obstruction. No hazards observed.

No deficiencies observed during today's inspection.
