Facility Evaluation Report
On 06/18/2026, Licensing Program Analyst (LPA) J. Duarte conducted an unannounced Annual Required Inspection. LPA met with Jeremiah Heitman, Director of Program Operations and Erlinda Gomez and stated purpose of visit.
LPA Duarte observed entire interior of building to require key card to access.
LPA toured the day program area and day program office during facility tour. LPA observed adequate seating and lighting available for clients. All furniture observed to be in good repair. Program is a community inclusion program, all clients were in the community at time of inspection.
Staff indicated that clients bring or purchase their own food. Staff does not administer medication.
LPA observed facility to be equipped with pull stations, strobe lights, and fire sprinklers through out building. Fire inspection was completed on 04/22/2026 and sprinkler system was inspected in March of 2026. Fire extinguisher observed to be present with a service date of 03/13/2026. A first Aid kit is hung on the wall.
A sample of staff files were reviewed. A sample of resident files were also reviewed and observed to have all required documentation.
No deficiencies issued during this visit.
An exit interview was conducted and a copy of this report provided for facility records.
