Facility Evaluation Report

On 05/13/2026, Licensing Program Analyst M Vega arrived at the facility unannounced to conduct an annual inspection. Facility was locked. LPA contacted Administrator Harjeet Kaur and left a voicemail. Administrator contacted LPA via telephone. During the visit Administrator was not able to attend, however, Spoke with CFO - Tigran Arutyunyan could respond to the facility to assist with the visit.

LPA toured the facility. Facility currently does not have any clients. Facility was clean and had several activity rooms with activities for clients. Facility has cleaning supplies in a locked storage room. Medications will be locked in a locked cabinet. There was seating and tables for clients in each room. The facility has tables and chairs set up in one room. Facility has appropriate lighting. Facility was at a comfortable temperature; temperature measured at 71 degrees F. Fire Extinguisher had a service date of 05/13/2025

Carbon monoxide and smoke detectors are operational. Facility smoke detectors and carbon monoxide are hard wired with sprinkler system. Facility has pull station fire alarm. Facility has required posting in the front receptionist area. Facility has a set of lockers in the front for clients to lock client's personal belongings.
