Facility Evaluation Report
On 06/13/2026,, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required visit. LPA introduced self, stated the purpose of the visit, and allowed entrance by direct care staff. Licensee, Olivia Roura was contacted by telephone and arrived a short time later to conduct facility visit.
Currently, there are 5 residents in care. All were present during inspection and observed to be relaxing in the family room watching television. The facility was observed to be at a comfortable temperature, clean, and free of odor. LPA observed adequate seating available for all residents in common area. Resident bedrooms toured, LPA observed resident's bedroom to have required furnishing available. Bathrooms observed to have grab bars, shower chairs, and slip resistant mats available. Bathroom fixtures observes operational, hot water temperature was tested at 109 degrees F. Kitchen toured, LPA observed the sharps and knives to be locked and secured in kitchen drawer. Facility observed to have a 2-day supply of perishable food and a 7-day supply of non-perishable food available for residents. Medications are locked and secured in hallway cabinet. Medications reviewed observed to have original labels and to be administered as prescribed. Fire extinguisher present with a service date of 4/30/2026, carbon monoxide and smoke detectors tested and observed operational at time of inspections.
Outside of facility toured. Side gate was self-closing and self-latching. Adequate outdoor seatings observed available for residents. All exits observed to open freely and are free of obstruction.
LPA reviewed resident and staff files during inspection. LPA received a copy of facility liability insurance and current copy of administrator certificate during facility visit. LPA requested the following documents to be submitted to Fresno Regional office no later than 7/03/2026: LIC 500, LIC 9020 and current copy of lease agreement.
No deficiencies cited during inspection. Exit interview conducted and a copy of report provided to Licensee for facility records.
