Facility Evaluation Report
On 6/11/2025, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required Inspection. LPA arrived, stated purpose of visit, and allowed entrance by Administrator, Rodelio Roura. Licensee/Administrator Olivia Roura contacted by telephone and arrived a short time later to conduct visit.
Facility tour conducted with Administrator. Facility observed to be well lit, odor free, and comfortable temperature. LPA observed residents engaging in activities at the dining room table at time of inspection visit. Facility living room and dining room observed to have adequate seating available. Resident bedrooms toured, bedrooms observed to have all required furnishings. Resident bathrooms toured, water temperature measured at 107 degrees F. Bathrooms observed to have grab bars in the shower and near toilet. Showers have shower chair and skid resistant mat available. LPA observed kitchen to have all sharps and utensils locked and secured in kitchen drawers. Facility observed to have a 2-day supply of perishable food and a 7-day supply of non-perishable food available. Medications are centrally stored and locked in hallway cabinet. All medications observed to have original labels and be administered as prescribed.
Outside of facility toured. All exits open free of obstruction. All exits open free of obstruction. No hazards observed. Facility has seating and a shaded area for residents.
Fire extinguisher present with a service date of 3/25/2025. Smoke and Carbon Monoxide detectors present and observed operational. Last Fire drill conducted 4/11/2025 according to facility records.
No deficiencies were cited during this inspection. Exit interview was conducted and a copy of this report provided for facility records.
