On 5/09/2026, Licensing Program Analyst (LPA) M, Medina conducted a Case Management visit due to observation made during complaint ( #24-AS-20260211162119 ) visit on this date.
Upon arrival to facility, LPA observed that the green waste receptacle in front of facility was broken and did not have a lid.
During visit, LPA contacted Licensee to advise of observation for a replacement to be requested.
Exit interview conducted and a copy of this report was emailed to facility for their records.
