The inspector’s account

On 10/22/2025 Licensing Program Analyst (LPA) B. Miranda arrived to the facility unannounced to conduct an annual inspection. LPA met with Administrator Charles Heard and was allowed entrance into the facility.

Administrator stated majority of the clients are in the transition program (19-22 years old) at the high school and attend program after school. LPA observed the facility to be clean, clutter free, and odor free. The facility is currently licensed for 60 individuals and has a current census of 13. Administrator stated medication is not kept at the facility.

LPA observed water temperature in a bathroom which read at 120 degrees Fahrenheit. Bathrooms are clean, odor free, and in working order. LPA observes various activities for the clients. LPA observed one client at the facility and during the visit another client was dropped of by their parent.

Smoke and carbon monoxide readers were tested and are in working order. Fire extinguisher was last serviced 6/5/2025 and still has charge.
