Facility Evaluation Report
On 04/02/25, Licensing Program Analyst (LPA) Yang arrived unannounced conduct a Case Management- deficiency visit. LPA introduced self, stated the purpose visit, and met with Administrator Adina Alaniz and Facility Social Worker Isaac Gomez.
The purpose of the today's visit is to address a medication error. On 03/20/25, the facility reported, facility staff did not sign the MAR after administering medications to C1 on 03/18/25 which resulted in the client receiving the same medication twice within one week. Medication is prescribed to be injected once a week.
A deficiency is being cited, per California Code of Regulations, Title 22, Division 6, see attached Lic 809D. A civil penalty is being assessed see attached Lic 421IM.
An exit interview was conducted. A copy of this report and appeal rights was provided to Administrator, whose signature confirmed receipt of report.
