Licensing Program Analyst LPA Shawna Doucette arrived at the facility unannounced to conduct a case management regarding an incident that occurred on 9/12/25. LPA met with Administrator Silvano Maldonado.
During the course of the investigation, it was found C1 did not want to go to Day Program. S1 forcibly picked up C1 and took C1 to the van to go to day program.
A copy of this report was provided to the Administrator along with appeal rights and plan of correction.
