Facility Evaluation Report
On 06/26/2026, Licensing Program Analyst (LPA) M Vega arrived unannounced to conduct a Case Management visit. LPA was granted entry by staff at facility. Administrator was contacted and arrived a short time later. LPA met with and explained the reason for the visit with Administrator - Ashley Voight.
Administrator self reported by submitting a Special Incident Report (SIR) on 06/16/2026 resulting in resident Client 1 (C1) not getting PRN medication that day. However C1 visited Physician practitioner and was able to obtain a refill on PRN medication.
During this visit, LPA reviewed the facility MAR, Centrally Stored Log and medications. LPA requested copied of reviewed documents as well.
There were no citations during this inspection.
An exit interview was conducted and a copy of this report was signed and provided.
