The inspector’s account

On 06/02/2026, Licensing Program Analyst (LPA) M Vega arrived to the facility unannounced to conduct a case management visit based on incident reports for Client 1 (C1) and Client 2 (C2). LPA was greeted by Staff 1, stated the purpose of the visit and was allowed entry into the facility. LPA met with Administrator (AD) - Mandy Wright.

Facility self-reported incident to CCLD. LPA requested a copy C1's Intervention Behavior Support Plan and C2’s Intervention Behavior Support Plan. LPA also reviewed CPI Cards, Cards are current and valid.

No deficiencies were observed and cited during this visit.

Exit interview conducted. Report was signed and Copy of this report was provided to AD for facility records.
