The inspector’s account

On 06/21/25, licensing Program Analyst (LPA) L. Salazar arrived at the facility unannounced to open a complaint investigation. LPA was greeted by staff, stated the purpose of the visit and was allowed entry into the facility. LPA met with Administrator.

LPA toured the outside of the facility. LPA observed perimeter fence surrounding the property that has a drive through gate in the front of the property, that opens and closes by key pad and/or remote. The front gate requires a phone call to the facility in order to obtain access to the facility is it is closed. A phone number is posted on the mailbox to call for access/entry. There is a walk-in gate identified on the sketch as an emergency exit but has no signs identifying the emergency exit on the gate and the mailbox is located directly in the path of the emergency exit gate.

On today's visit, the front gate was open and the house was accessible from street. The front drive gate was observed to be disabled / turned off at the time of visit. The front drive gate will remain open at all times unless a fire clearance request is requested and approved. LPA provided technical assistance to Administrator.

An exit interview was conducted with Administrator. A copy of this report was discussed and provided via email. No deficiencies cited on today's visit.
