Facility Evaluation Report

On 05/01/26, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct a Case Management visit for closure and met with Administrator Julie Juarez and Executive Director Tamara “Tammy” Baker and Director of Family Services and Program Otilia Benavidez. The department received notification from the facility of forfeiture license. The purpose of this visit was to conduct a final walk through the facility. No staff and clients were observed present.

LPA toured the facility inside with Administrator, Executive Director, and Family Services and Program Director. There was no indication of program in session in the facility. Last and final program session was conducted on 04/30/26. All clients were notified and effective 04/30/26 there are no longer programs provided at the facility.

Forfeiture to be sent. The facility will be closed effective 05/01/26, pending Licensing Program Manager

approval. Exit interview conducted. Executive Director was provided a copy of this report.
