On 12/4/2025 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to conduct an unannounced annual inspection. LPA was greeted by staff and was informed Administrator would not be available until after 1:30. Staff also informed LPA a resident had active Covid.
As a precaution LPA did not conduct annual inspection and will return to conduct at a later time.
