The inspector’s account

Licensing Program Analyst (LPA) J. Duarte, arrived at the facility unannounced to conduct the Required Annual Inspection. LPA stated the purpose of the inspection and was granted entry into the facility by Staff Deanna Avila. Staff contacted Administrator Beatrice Villegas who responded to the facility to assist with the visit.

A tour of the facility was conducted with the Administrator. The residence was set at 78 degrees F and free of passageway obstructions inside and outside. There were three residents at the facility and staff stated that one resident was at day program.

LPA observed 4 bedrooms in the residence. Residents' rooms were toured and inspected. Rooms were found to be clean with required furnishings.

Kitchen was toured. Facility was observed to have a two day supply of perishable and seven day supply of nonperishable food. The knives were stored in a locked drawer. Medications were stored in a locked closet in the hallway. Cleaning supplies were in a locked cabinet in the garage. Smoke detectors and carbon monoxide detectors were checked and observed operating during this visit. Fire extinguishers were charged and had service dates of 11/20/24. On 08/04/25, per staff records, a fire drill was conducted.

LPA observed a self latching gate on the outside of the residence next to the garage. There was outdoor seating for the residents underneath the patio immediately exiting the den. The facility has a pool which is fenced and locked, inaccessible to residents.

LPA reviewed a sample of medication and the MARS, and medication was observed to be administered as prescribed. All resident records and a sample of staff records were reviewed and found to be complete.
