Facility Evaluation Report
On 08/20/2025, Licensing Program Analysts (LPA) M. Medina arrived at the facility unannounced to conduct the annual required inspection. LPA was greeted by the Administrative Assistant, Sean Heredia stated the purpose of the visit and was allowed entry into the facility. Administrator, Christine Williams contacted by telephone and arrived a short time later to conduct visit.
LPA toured the facility inside and out with Administrative Assistant. LPA observed the facility to be clean, free from odor, and a comfortable temperature.. Five individual classrooms toured, all clients observed to be engaged in educational activities with staff. Staff ratios observed to be 3:1 with the exception of individuals who require 1:1 supervision. Client bathrooms toured, water temperature measured at 114 degrees F. No meals are prepared on site, clients bring meals from home. Classrooms rooms observed to be equipped with small refrigerators and microwaves to store and or heat up food if necessary. Disinfectants and cleaning supplies were observed to be locked in a utility cart in the hallway and inaccessible to clients. First Aid kit were observed to have the required items.
Emergency disaster plan and infection control plan on site and available. Facility sketch was observed to be posted. Last fire drill was conducted on 06/18/25. Doors and passageways were observed to be free from obstruction throughout the program. Fire extinguishers were observed with a service date of 10/04/2024.
LPA requested the following forms to be updated and submitted to Fresno Regional Office no later than 9/05/25: Designation of Facility Responsibility (LIC308), Administrative Organization (LIC309), Personnel Report (LIC 500), Client Roster (LIC 9020) , and Emergency Disaster Plan (LIC 610D).
