Facility Evaluation Report

Licensing Program Analyst's (LPA's) Carmen Lopez, and Jose De La Cruz conducted an announced case management visit to observe the health and safety of the residents in care. LPAs were greeted, granted entry into the facility, and met with temporary Administrator Robert Cantoria.

On January 10, 2026, the RM and LPM met with the Licensee. The Department issued a Temporary Suspension Order (TSO) to the former administration, and a temporary Administrator was contracted to assume facility's operation for a period of 90 days.

During today’s visit, LPAs Lopez and De La Cruz toured the facility and verified the health and safety of residents in care. The LPAs also met with staff to explain the TSO and answer questions. LPA's toured the facility,and conducted a health and safety check for the residents in care. LPA's requested and obtained relevant information.

An exit interview was conducted, and a copy of this report, along with Licensee Rights (LIC 9058, 03/22), were provided to temporary Administrator Cantoria at the conclusion of the visit. The signature below confirms the documents were received.
