The inspector’s account

Licensing Program Analyst (LPA) David Roman conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPA identified himself to House Manager, Jason Armstrong. LPA D. Roman discussed the purpose of today's visit with House Manager, Jason Armstrong. According to the facility’s license, the facility has a maximum capacity of six clients, of whom one maybe non-ambulatory. At a later time Administrator, Elizabeth Waycott arrived to the facility.

LPA along with House Manager toured the interior and exterior of the facility and inspected each room. The facility was sanitary, free of obstructions, and in good repair. Pathways were clear of slip hazards. Resident bedrooms contained the required furnishings. Doors, windows, toilets, and showers were in working order. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and resident activities.

Cooking/dining equipment and utensils were present. There were no toxic chemicals/poisons accessible to residents in care. Medications were labeled, as required, and stored in locked cabinets. Water temperature was measured at 105 degrees F. Refrigerator was fully stocked with food lasting about 7 days, non-perishables were checked for valid expiration dates.

No pools or bodies of water on the premises. Per House Manager no firearms or ammunition are kept at the facility. Fire alarm and carbon monoxide detectors were tested at 2:15 PM. The facility has functioning telephone and internet services. First Aid kits and emergency lighting readily available. Fire extinguisher(s) were serviced on 03/2025 and present within the facility. Resident records reviewed had required documentation. Staff records reviewed also had required documentation.

An exit interview was conducted with House Manager, Jason Armstrong, to whom a copy of this report and the Licensee/Appeal Rights were provided during the visit. Their signature acknowledges the receipt of this report and their rights.
