Facility Evaluation Report
Licensing Program Analyst, David Roman (LPA D. Roman) conducted an unannounced case management visit to process the facility's closure. LPA D. Roman gained entry upon arrival to the facility by identifying himself to and discussing the purpose of the visit with Administrator, Siklalic Garcia. LPA D. Roman had previously spoken to Licensee, Quetzali Kahnis, by phone and advised the purpose of the visit.
During today's visit, LPA D. Roman along with facility staff conducted a tour inside and outside of the facility. LPA D. Roman inspected each room and observed the facility to be vacant. The home is empty of resident property however, some household furnishings are present (table, chairs, couches, books, and nightstands).
On May 21, 2025, the Licensee notified the Department of the facility’s closure process. At the time of notification, the facility had a census of 12 residents. The residents were relocated to their sister facilities, Majestic and Sunset Villa. Licensee had previously advised POA’s of plans to relocate residents. No deficiencies were issued during this visit and the facility is ready for closure.
An exit interview was conducted with Administrator, Siklalic Garcia, and with Licensee, Quetzali Kahnis via phone. Licensee was not present in person to sign this report.
