Facility Evaluation Report
Licensing Program Analyst (LPA) David Roman conducted an unannounced case management visit to follow up on an incident report involving physical aggression and eviction. LPA identified himself and was granted entry by Jose Navarro, Office Manager, and Program Administrator, Carolina Nunez-Garcia. LPA stated the purpose of the visit and reviewed the basic elements of this visit with Program Administrator.
The Department conducted an investigation for an incident which transpired at the social rehabilitation facility on May 04, 2026. It was confirmed that client (C1) had been the victim of a physical aggression by their roommate/perpetrator (C2) during the nocturnal shift which resulted in law enforcement involvement leading to the voluntary discharge of the perpetrator (C2). Program staff contacted emergency services and C1 was taken to the hospital for further evaluation.
During today’s visit, LPA conducted an interview with the Administrator and conducted a record review, regarding the incident for additional information. LPA provided the Administrator with guidance to the facilities client handbook, section of client rules and expectations, pertinent to possible eviction procedures compliant with Title 22 regulations of 3-day evictions (Department approved) and 30 day eviction notices.
Based on the information obtained during the visit, deficiencies were not observed or cited during this CM visit.
The report was discussed and an exit interview was conducted with Administrator, Carolina Nunez-Garcia to whom a copy of this report along with Licensee/Appeal Rights were provided at the conclusion of the visit. Their signature below confirms the receipt of their rights and this report.
